  1. This document sets out the safety requirements you must follow when operating a tower crane on behalf of JIA AN Construction Pte. Ltd. 本文件列明你代表嘉安建筑操作塔吊时必须遵守的安全要求。
  2. Read every section. You must confirm each section individually before you sign. 请阅读每一节。签署前须逐节单独确认。
  3. If any requirement is unclear, ask your supervisor before confirming. Do not confirm anything you do not understand. 如有任何要求不清楚，请先向主管询问再确认。不理解的内容不要确认。
  4. The English text is the governing version. The Chinese text is provided to assist understanding. 英文为正式文本，中文供理解参考。

What this document is. It is a record that these safety requirements have been issued to you and that you have read and understood them. It is not a waiver. It does not transfer the Company's legal duties to you, and it does not reduce any right you have under law.

本文件的性质。它是一份记录，证明公司已向你发出上述安全要求，且你已阅读并理解。它不是免责协议，不将公司的法定义务转移给你，也不减少你依法享有的任何权利。

Operator Particulars

操作员资料
What the Company records. Your name, contact number, FIN or work permit number, handwritten signature, and which sections you confirmed are sent to JIA AN Construction Pte. Ltd. and stored on a server in Singapore. They are kept as the Company's record that this document was issued to you and acknowledged by you, and are used for no other purpose. They are not shared with anyone outside the Company except where the law or a main contractor's site requirements oblige us to produce them. To ask what is held about you, or to correct it, contact the Company at 8546 1360. 公司会记录什么。你的姓名、联系电话、FIN／工准证号、手写签名，以及你确认了哪些章节，会发送给嘉安建筑并保存在新加坡的服务器上。这些资料仅用于证明公司已向你发出本文件、你已确认，不作其他用途，也不会提供给公司以外的任何人——除非法律或总承包商的工地规定要求我们出示。如需查询或更正公司持有的你的资料，请联系公司 8546 1360。
